Note: You must have a professional mental health license and must have at least five years seeing couples (can include graduate school traineeships, internships, post licensure experience, etc.) Here is what else you will need:

Professional License Number (Remember if you don't have it handy you can look it up online.)
Months/years of clinical experience working with couples
Marriage/Couples Training workshops (workshops and presenters, don't worry if you don't have extreme details we just want to see what you've generally done)
